Please read over the following points carefully before completing the online survey. Please indicate if you give your consent to participate in the study by ticking each statement and completing the date at the end.

Please note that you can withdraw from this study at any time without giving a reason by closing the browser window before submitting your answers. You can also skip any items of the survey which you do not wish to answer.

I understand that my participation in this project will involve taking part in a short online survey. The purpose of this study is to make the existing Fishers’ Non-technical Skills (FiNS) system personalized and useful for specific roles (skipper and deckhand). The survey will involve evaluation of the skills, elements and behaviours identified in previous research. The survey will take approximately 20 minutes to complete.

I understand that participation in this study is entirely voluntary and that I can withdraw from the study at any time without providing a reason, by closing the browser window before submitting my answers. If I withdraw from the study, my data will not be included.

I understand that I can discuss any concerns with the researcher before or after participating.

This research is carried out by research fellow Dr Ilinca-Ruxandra Tone and senior lecturer Dr Amy Irwin in the School of Psychology, University of Aberdeen. The research project is funded by Trinity House.

I understand that the information in the survey will be collected and held anonymously so that it is impossible to trace the information back to any individual. The anonymous information will be stored on a secure server and may be retained indefinitely.

I confirm that the purpose, participation information, and data storage of this research have been sufficiently explained.
